The course explains why high-quality documentation matters, how ethical and legal requirements shape everyday practice, and what distinguishes strong clinical notes from common documentation errors. Learners explore standards for both direct and indirect session notes, and incorporate medical necessity principles. By the end of the course, clinicians will understand how to create session notes that support effective treatment, ethical billing, and continuity of care across service providers.
